CDCP Registration and Treatment Process

Step 1 – Registration

  • Apply for the Program

  • Approval & Welcome Package
    If eligible, Sun Life, the plan administrator, will send a package including your member card and coverage start date. Coverage usually begins within weeks of approval.

  • Covered Services: A variety of service options are available to meet patient needs, including check-ups, cleanings, fluoride, sealants, fillings, dentures (to help replace missing teeth), extractions, and some root canals
  • Pre-Authorization Needed: For crowns, surgical extractions, partial dentures, orthodontics
